Full Job Description

This is an exciting opportunity to join our Hospital Discharge Team, playing a vital role in supporting adults with care and support needs to achieve the outcomes that matter most to them. Working in a fast-paced and rewarding environment, you will undertake a range of assessments and interventions, working closely with individuals, families, carers, health colleagues and partner organisations to support safe, timely and person-centred hospital discharges. You'll be at the heart of delivering our Home First and Discharge to Assess approach, ensuring people spend no longer in hospital than necessary and are supported to recover, regain independence and remain connected to their communities. Rather than making long-term decisions in a hospital setting, you'll help ensure assessments take place in the most appropriate environment, focusing on people's strengths, what matters to them, and the support available within their families and communities. As part of a multidisciplinary team, you will work closely with NHS and partner agencies to facilitate seamless transitions from hospital, reduce delays, promote independence and improve outcomes for local people. We're looking for enthusiastic and motivated Social Workers who are committed to delivering high-quality, person-centred practice.,

  • Be a qualified Social Worker registered with Social Work England.
  • Have experience of working within Adult Social Care and a sound understanding of relevant legislation, hospital discharge guidance and best practice.
  • Be passionate about promoting independence, wellbeing, choice and control.
  • Have excellent communication, assessment and partnership-working skills.
  • Be committed to your own professional development and continuous learning.

    Salary Range: SCP 26 £37,280 - SCP 34 £45,091 pa Career Grade (Bar at £39,152 & £40,777 & £42,839) depending on experience Ready to Take the Next Step in Your Social Work Career?
