Are you an experienced social worker who enjoys assessment, analysis, problem-solving and improving outcomes, but is looking to move away from the demands of frontline statutory practice, high-risk case management, safeguarding enquiries and court work? The London Borough of Merton's Targeted Review Team offers an opportunity to apply your professional skills in a role focused on reviewing, evaluating and improving support arrangements for adults receiving services across Adult Social Care. About the Role As a Social Worker within the Targeted Review Team, you will work across Adult Social Care services undertaking reviews of individuals receiving support. Your work will help ensure services remain person-centred, proportionate and aligned with assessed needs, while promoting independence and positive outcomes for residents and carers. Using your professional knowledge and analytical skills, you will:

  • Complete strengths-based reviews and reassessments.
  • Evaluate support arrangements against assessed needs and outcomes.
  • Make evidence-informed recommendations regarding care and support provision.
  • Work collaboratively with residents, carers, providers and multidisciplinary colleagues.
  • Support effective decision-making and resource allocation.
  • Ensure care and support plans accurately reflect needs, strengths and goals.
  • Contribute to quality assurance and continuous service improvement.
