We are looking for enthusiastic and motivated Level 1 Social Workers to join our Hospital Social Work Team based at Lincoln County Hospital. Our team also supports patients across Northern Lincolnshire and Goole hospitals, including Grimsby, Scunthorpe, Hull and Goole, working closely with health colleagues to ensure safe, person-centred discharges.   • Manage a caseload with appropriate supervision, developing your professional confidence and autonomy
  • Undertake Care Act 2014 assessments, care and support planning, and reviews
  • Work collaboratively with individuals, families, carers and professionals to identify strengths and promote independence
  • Contribute to safeguarding enquiries, ensuring the person's voice and desired outcomes remain central
  • Work closely with health colleagues to support safe and timely hospital discharges
  • Identify wider needs such as housing, health or community support and work with partners to address them
  • Promote strengths-based practice and support people to build and maintain support networks
  • You will be supported by experienced practitioners and leaders, with opportunities to develop your practice, confidence and career progression within Adult Care.

    In accordance with Part 7 of the Immigration Act 2016 (Fluency Duty), the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.

  • Monday - Thursday: 8:45am - 5:15pm
  • Friday: 8:45am - 4:45pm
  • Our service also operates a weekend working rota, currently one weekend in four.
  • Weekend hours: 8:30am - 4:30pm
  • When you work a weekend, you will receive a day off during the week, helping to maintain a healthy work-life balance.,
  • Salary £35,412 - £39,152 with additional 15% attraction and retention payment until March 2026 (pro rata)
