Full Job Description

Vacancy of 2 x Social Worker Posts for the Discharge to Assess South Community Team. This is part of the hospital service area and namely focuses on the post discharge element of the Citizen's journey. The team covers the South of the City. We support the assessment and support planning for citizens who have left the hospital with a short term service and require support to determine the longer term support needed where applicable. There is also a sub team which covers Summer Hill Terrace which is part of the Homeless Pathway. Please refer to the Job Description for the duties and responsibilities expected in the Social Worker role. This is a full time post: Monday to Friday 36.5 hours a week There is an expectation to complete face to face visits and MDT (Multi Disciplinary Meetings). We are looking for a dedicated Adults' Social Worker to join our supportive and forward-thinking team. You will work with adults with a wide range of needs, including physical disabilities, learning disabilities, mental health conditions, and age-related challenges., Complete strengths-based assessments and develop personalised support plans Promote independence, choice, and wellbeing through creative interventions Safeguard adults at risk and coordinate multi-agency responses Support carers and families, ensuring their voices are heard

Qualified and SWE-registered Social Worker Strong understanding of the Care Act, Mental Capacity Act, Safeguarding knowledge and principles and wider legislation relevant to Adult Social Work Ability to build trusting relationships and work collaboratively Commitment to promoting dignity, independence, and positive outcomes.
