An exciting opportunity has arisen for a Social Worker to within the Mental Health Liaison Team (MHLT). MHLT are based within the Royal Stoke Hospital and see all patients who are referred to the service from A&E, emergency portals, inpatient wards and the wards at the Haywood Hospital.

This developmental post will be offered at a Band 5, with progression to Band 6 upon successful completion of the Assessed and Supported Year in Employment (ASYE). You will support and supervision by one of our Senior Social Workers

As a Social Worker you will work within a multidisciplinary, providing assessment, signposting and safety planning to those referred to the service., The post holder will be accountable on a day-to-day basis to the Team Leader, who will allocate, monitor and review all work through regular informal and formal supervision. The post holder will also be professionally supported by an experienced Social Worker

The post holder will have a key role to play in relation to supporting assessments and a range of interventions for service user those under the care of the team. The role will involve assessing; accessing; and implementing a range of interventions and providing guidance/advice and information for service users with acute or increasingly complex long-term conditions that could include mental health needs, learning difficulties or physical health difficulties.

The post holder will be required to be an integrated and supportive member of a team, planning, actioning, monitoring and evaluating work with the supervision of an experienced Social Worker and the Team Leader.

The post holder will be responsible for supporting the delivery of assessment and intervention for service users withinthe directorate and where relevant Hospital/ Inpatient settings providing social work knowledge and expertise. The post holder will support with requests for information, advice, guidance, with referrals, initial assessments and interventions.

The post holder is responsible for working collaboratively with other Trust services to provide seamless interventions for service users and carers.
