Are you a passionate and qualified Social Worker looking to make a real difference in urgent care settings? Join our dynamic Discharge to Assess Team , a frontline service working collaboratively with the hospital and supporting citizens from Birmingham's most deprived communities. About the Discharge to Assess Service Our team plays a vital role in:

  • Supporting patient flow across the hospital system
  • Identifying community-based solutions
  • Signposting and advising citizens and carers
  • Conducting Care Act and Mental Capacity assessments
  • The team help improve health outcomes and experiences for citizens in Birmingham

    We're seeking Qualified Social Workers who:
  • Hold a CQSW, DipSW, CSS or equivalent Social Work England-recognised qualification
  • Registered with Social Work England
  • Have experience working with vulnerable adults (hospital experience is desirable but not essential)
  • Can work effectively in a fast-paced, multidisciplinary environment
  • Is confident in making decisions under pressure and managing complex needs
  • Is committed to equality, continual professional development, and service improvement
