Full Job Description
Due to recent developments, Urgent care are looking to recruit a social worker who will provide social work support, strength-based assessments, tailored support planning and reviews in line with the Care Act 2014 for adults across Trafford., In return we offer flexible working arrangements, provide opportunities to develop skills through a structured development programme, and provide regular support through professional supervision.
You will need to hold a relevant social work qualification and be registered with Social Work England. We are looking to appoint enthusiastic individuals to undertake strength-based assessments and deliver appropriate person centered support and outcomes for adults with care and support needs. You must be able to work in ways that promote independence, recognise the potential for change and through excellent practice enable people to maximise their strengths. About You You must possess good communication skills and be able to work proactively with a range of adults applying a clear understanding of the legislative framework for Adult Social Care. In addition you will be required to provide an effective response to safeguarding enquiries in line with both the Care Act 2014 and local policy and procedures. The successful candidates will be able to work as part of a team and autonomously and will be familiar working within a Multi-Disciplinary environment. Further, they will have knowledge of working within the Mental Capacity Act 2005 and will be able to effectively and professionally challenge to ensure best outcomes for our residents.
