Full Job Description

Under the umbrella of Safeguarding, you will:

  • Undertake strengths based social care assessments, care and support planning, and reviews in line with statutory responsibilities
  • Manage a caseload that promotes choice, independence and wellbeing
  • Contribute to safeguarding enquiries and best interest decision making
  • Work collaboratively with health, housing, voluntary and community partners
  • Maintain accurate and timely records using electronic case management systemsYou will be supported through regular supervision, reflective practice and ongoing learning, enabling you to develop your skills with confidence.

    Hartlepool Borough Council is looking to recruit a motivated and committed Social Worker to join our Learning Disability Team within Adult and Community Based Services. This is an excellent opportunity to work within a supportive, experienced and forward thinking team that is passionate about delivering high quality, person centred outcomes for adults with learning disabilities., Is a qualified Social Worker registered with Social Work England
  • Has recent and relevant experience within adult social care (desirable within learning disabilities)
  • Has sound knowledge of key legislation, including the Care Act, Mental Capacity Act and safeguarding frameworks
  • Demonstrates strong analytical and decision making skills within appropriate timescales
  • Is committed to person centred, strengths based practice and works with sensitivity, professionalism and compassion
