Full Job Description
We are looking for 2 Social Work Assistants to join our Wellbeing & Access Hub, the first point of contact for adults and carers accessing support.
This is a varied and rewarding role where you will work alongside Outreach, Carers IW and Occupational Therapy colleagues to help people access timely advice, short-term support and reablement services, reducing the need for long-term care.
You will take referrals, complete assessments under the Care Act, carry out visits and offer practical information, advice and signposting. Your work will promote independence, choice and wellbeing, while also supporting carers and families, including completing Carers Assessments where needed. You will contribute to safeguarding practice, attend discharge planning meetings and maintain clear, accurate records.

You will be suitably qualified with up-to-date professional registration and have a good working knowledge of the Care Act 2014, Mental Capacity Act 2005 and data protection requirements.
Occasional participation in the Out of Hours stand-by rota may be required.
