Full Job Description

As our new Social Prescriber your responsibilities will vary. You will

  • Complete initial assessments with patients referred to and eligible for the social prescribing service. Assessing their current wellbeing and identification of what matters to them.
  • Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing.
  • Manage a caseload of social prescribing patients providing follow-up support and ensuring they are able to access prescribed services with confidence.
  • Be responsible for the scheduling and sequencing of patients social prescribing activities.
  • Liaise with Health and Prison departments, to develop awareness and support delivery of the service.

    Salary This position is full time, youll receive an annual salary up to £37,741 per annum - depending on experience.
