We are seeking a motivated and dedicated individual who is committed to delivering high-quality, person-centred support and working collaboratively as part of an integrated team. In this role, you will provide essential support to a busy social work team, helping to ensure that adults across Stockton-on-Tees receive effective assessments, care management, and timely reviews. You will work in line with current legislation, government guidance, and departmental policies and procedures to support the delivery of safe and responsive services., Accuracy and Use of AI Tools: You must ensure that anything submitted in your application is factually accurate. You will work closely with individuals, their families, and a wide range of professionals and partner agencies outside the Council. The role involves undertaking assessments and developing support plans, with a strong focus on accurate and timely record keeping. Excellent communication skills - both written and verbal - are essential, alongside the ability to build positive relationships and work effectively as part of a team.
This is a part time role of 30 hours per week and the actual salary for the role is £25,995 - £27,323., We offer an excellent total reward package, including:
