Full Job Description
As a Social Care Officer within the East Neighbourhood Social Work Team, you will play a key role in supporting adults and their families to live independently, build resilience and achieve the outcomes that matter most to them. You will undertake person-centred assessments and support planning under the Care Act 2014, including assessments for basic equipment and Technology Enabled Care (TEC).
Working as part of our neighbourhood team, you will contribute to a responsive duty service, help residents access community support and services, and work collaboratively with health, housing and voluntary sector partners to promote positive outcomes and Gloriously Ordinary Lives for the people of Stockport., This post is also open as secondment opportunity for Stockport Council Employees, however, you must seek the permission of your current manager prior to making an application.
The successful applicant's appointment will be subject to satisfactory pre-employment clearances including a Disclosure and Barring Service check.
Stockport Council holds a Visa sponsorship licence but this can only be used for certain roles and this campaign does not currently qualify.
