Full Job Description
Connect to Community is Surrey's single front door for adult social care, supporting residents through timely, strengths-based responses following initial triage. C2C brings together professionals from different disciplines to ensure residents receive the right support, at the right time, with fewer handovers. Social Care Assistants in C2C play a critical role in:
- Supporting service users to be able to be in more control of their care, making informed choices and being active citizens in their community
- Supporting consistent, high-quality practice across the team
- Working closely with partners across health, housing, reablement, TEC and the voluntary sector. Daily multi-disciplinary meetings provide a strong forum for shared learning, professional challenge and collaborative risk management. About the Role Connect to Community is a fast-paced, front-door environment where, as a Social Care Assistant, you will have frequent direct contact with the public both over the phone and when visiting individuals in their own homes or care settings. In this role, you will undertake assessments, reviews and targeted interventions in line with the Care Act, while also taking on safeguarding responsibilities, including Section 42 enquiries. You will promote a recovery-focused, strengths-based approach, supporting individuals to achieve positive outcomes and maximise their independence. Applying sound professional judgement, you will manage risk, uncertainty and competing priorities effectively, and will develop and implement strengths-based support plans that enhance wellbeing, choice and control. You will work collaboratively with individuals, carers, multidisciplinary professionals and service providers to ensure coordinated and effective support for vulnerable adults and their families., 1. Looking at our advert, and considering the Council's commitment to ensuring that "no one is left behind", please tell us what it is about the role that has motivated you to apply? 2. Please tell us about the relevant experience or transferable skills you have that you can bring to this role? 3. What three qualities do you have that would make you a good Social Care Assistant and how do these qualities support Surrey County Council's culture of collaboration, inclusivity, and adaptability? Before submitting your application, we recommend you read the job description & Our Life at Surrey handbook to get an insight into working at Surrey. An enhanced DBS 'Disclosure and Barring Service' check for regulated activity (formerly known as CRB) and the Children's and Adults' Barred List checks will be required for this role.
- Your understanding of Adult Social Care needs and any experience you may have in this area. It doesn't necessarily have to involve visiting people in their homes or in care settings
- Your ability to manage a caseload efficiently, regularly monitoring support plans for individuals and carers
- Experience of working directly with the public on the phone or in person
- Your ability to build relationships with various stakeholders, colleagues, and the public effectively
- Competence in a range of IT skills and a willingness to learn to use a variety of software systems
- Good communication skills, with the ability to convey information clearly, concisely, and respectfully, both verbally and in writing
- A commitment to your own professional development
- The ability to empathise with, and show compassion towards, individuals in need of social care assistance. Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.
- 26 days' holiday, rising to 28 days after 2 years' service and 31 days after 5 years' service
- Option to buy up to 10 days of additional annual leave
- A generous local government salary related pension
- Up to 5 days of carer's leave and 2 paid volunteering days per year
- Paternity, adoption and dependents leave
- An Employee Assistance Programme (EAP) to support health and wellbeing
- Learning and development hub where you can access a wealth of resources
- Wellbeing and lifestyle discounts including gym, travel, and shopping
- A chance to make a real difference to the lives of our residents
