Full Job Description

This is an exciting opportunity for an experienced care professional who is ready to take the next step in their career. As a Service Coordinator, you will play a key role in ensuring the delivery of safe, high-quality, person-centred care. You will coordinate and motivate a vibrant team of Care staff, ensuring excellent outcomes for our learners and citizens while maintaining compliance with regulatory requirements and standards. You will: 1. Lead and support a team of approximately 15 to 20 care staff. 2. Deliver and oversee person-centred care planning and support. 3. Ensure safeguarding practices and Mental Capacity Act principles are embedded within service delivery. 4. Administer medication and PEG care in accordance with organisational procedures. 5. Monitor and review learner and citizen progress against agreed goals and outcomes. 6. Build effective relationships with families, advocates and healthcare professionals. 7. Conduct staff supervision and annual performance reviews. 8. Support recruitment, induction and workforce development activities. 9. Contribute to quality assurance processes and continuous service improvement. 10. Act as Site Coordinator on a rota basis.

1. Excellent verbal and written communication skills. 2. Experience in supporting people with learning disabilities, autism, physical disabilities and complex needs. 3. Basic IT skills, including Microsoft Office and report writing. 4. The ability to prioritise workloads and meet deadlines. 5. Experience working effectively as part of a team. 6. Strong interpersonal skills and the ability to build positive relationships. 7. A demonstrable understanding of safeguarding practices, GDPR and confidentiality requirements. 8. Knowledge of CQC Fundamental Standards. 9. Experience of delivering staff training and coaching., Essential 1. Level 3 NVQ in Health and Social Care 2. Level 3 NVQ in Leadership & Management Desirable 1. Level 2 English 2. Level 1 Mathematics

    This role operates on a flexible shift pattern across seven days: 1. 6:45am to 2:45pm 2. 2:00pm to 10:30pm
