Summary


We are looking for a creative and resourceful Social Care Service Co-ordinator to work with us in the Learning Disability and Neurodiverse Team which is part of the Adult Services and Wellbeing Directorate at Calderdale Council. Our role is to work with and support adults from the age of eighteen to access support in the community and your support will mainly be within our Preparing for Adulthood pod.


We work in partnership with a variety of people who may also have a dual diagnosis, their families, carers and other agencies and ensure they have a fulfilled life.


We use strengths-based practice focussing on positive outcomes and being independent and strive to put the person at the heart of assessments.
