The Discharge to Assess/Intermediate Care at Home teams are looking for a Senior Support Worker, to fulfil an exciting opportunity within the community in Stockport NHS Foundation Trust We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries. An opportunity has arisen for a Band 3 Senior Support Worker, to work flexibly within the Discharge To Assess/Intermediate Care at Home Team. The Discharge To Assess element is a multi-professional service who assess patients upon discharge from hospital in the community. We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies. The Intermediate Care at Home Team provide nursing and therapy rehabilitation to patients within the Stockport community. This role will require the post holder to be flexible to cover all areas within the Discharge to Assess service, covering 7 days 08.00-21.00Main duties of the job To work within the multi-disciplinary integrated care teams that deliver community based services in the Discharge to Assess and Intermediate Care at Home, undertaking nursing, therapy and social care tasks within agreed parameters, delegated by the registered practitioners. The job will include home visiting and lone working without direct supervision. The scope of the role will focus on supporting early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions.

  • To provide nursing, therapy and social care to patients within their own homes, carrying outhigher level delegated clinical, therapeutic and personal care intervention.
  • To promote the physical, mental and emotional health and wellbeing to individuals and client groups.
  • To be an effective member of the Multidisciplinary Team.
  • To communicate effectively with patients and carers, recognising their needs for alternative methods of communication
  • To carry out care/other duties under indirect supervision of a registered professional.
