Care Plus Groups Community Learning Disabilities Team provides specialist, person-centred community support for adults with a learning disability across North East Lincolnshire. The service works in partnership with individuals, families, carers, providers and wider health and social care colleagues to promote independence, safety, meaningful occupation and improved quality of life.
The team delivers specialist assessment, intervention, advice and care planning for people with learning disabilities who may have complex physical health, functional, sensory, behavioural, environmental or social care needs. The post holder will contribute to a multidisciplinary and multi-agency approach, ensuring Occupational Therapy input is aligned to the individuals goals, legal frameworks, safeguarding responsibilities and Care Plus Group policies and values.
We are seeking to recruit a passionate, committed and highly motivated occupational therapist to join our well-established and innovative community learning disability team at Care Plus Group.
You will be joining a friendly, supportive and creative multi-disciplinary team, dedicated to achieving great outcomes for adults with learning disabilities., As the Lead Occupational Therapist you will provide support to ensure people are able to live an enriched life and be as independent as possible. They are a person-centred service offering a wide variety of support such as accessing health promotion groups, working on road safety, enable individuals to complete activities of daily living more independently, improving mobility, supporting individuals to meet goals around finances and relationships and ensuring individuals have appropriate housing and this is adapted to meet their needs.Working closely with GP's, practice nurses, hospital staff and a wide range of other professionals and agencies, with an aim of reducing inequalities for the individuals that access the service.
You will have will have a passion for improving the lives of adults with learning disabilities, collaborating with colleagues and across services to deliver results, and be passionate about developing their own skills and those of colleagues. You will need to be able to manage your own diary with the ability to adapt and be flexible to suit the needs of the service.
