Senior Respiratory Physiotherapist

Working as part of the Integrated Respiratory Team to provide high quality, evidenced based, timely care for patients that fit service criteria. The Integrated Respiratory Team includes COPD Team (complex respiratory case management), Pulmonary Rehabilitation Service, Home Oxygen Service Respiratory Virtual Ward and Breathlessness and Diagnostics Service (Spirometry) as a whole unit. Staff are expected to support all aspects of service delivery. Lead in the provision of Pulmonary Rehabilitation for suitable patients requiring help with respiratory condition management including but not limited to COPD/ Pulmonary Fibrosis and Bronchiectasis. Working with the patient and providing individualised rehabilitation, self-management and independence throughout the continuum of care. Completing respiratory assessments and reviews within the COPD team for ongoing respiratory case management. Supporting the home oxygen service. Providing and partaking in care and support for the Respiratory Virtual
Ward, giving care closer to home and supporting admission avoidance and early supported discharge pathways. Performing Spirometry and FeNo testing as part of Breathlessness and Diagnostics service, and any future service adaptations. Providing onward referrals to other community services including health and social care to ensure the safe management of the patient.

Main duties of the job

Lead in the review and development of clinical policies, clinical pathways and identify improvements to service provision. To be highly skilled in supporting COPD and other respiratory patients in the community, utilising highly developed specialised knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience, and competence Work as part of a multi-disciplinary team providing community based holistic assessments and participate in other areas of service provision within the Integrated Respiratory Team. Maintain a high standard of patient focused care reflecting an inter-disciplinary approach to rehabilitation and self-management of patient cohorts. Providing professional intervention for community management to prevent unnecessary hospital admissions and improve quality of life. To perform quality assured spirometry and interpretation of results, including registration to ARTP body. Establish and promote joint working with other services across
professional and organisational boundaries and to be able to manage complex and sensitive situations with patients and professionals. Makes complex judgements requiring analysis, interpretation, and comparison of options by integrating complex knowledge sources in new and/or unfamiliar contexts. To have organisational knowledge relating to Trust protocols and procedures and adhere to them. To be responsible for providing accurate records of information required by the Trust for audit purposes
