Full Job Description
An exciting opportunity has become available for a Band 4 Senior Rehabilitation Assistant to join the therapy team at Trafford Hospital.
You will work flexibly across our inpatient clinical areas supporting our well-established therapy teams, alongside Clinical Lead therapists, Physiotherapists and Occupational Therapists.
In this role, you will assess and support patients across a range of specialties, including Stroke, Hip Fracture, Neuro-rehabilitation, Medicine and Orthopaedics. You will deliver exercise and rehabilitation interventions to support an active hospital approach and help patients regain independence. You will also complete a small amount of day-to-day administrative work to help the service run smoothly.
We provide therapy services across 7 days, so all staff are expected to take part in the weekend rota.

Independently complete assessments and deliver agreed interventions for a designated caseload of patients with routine clinical needs, using established treatment plans and pathways.
Support patients with more complex needs under the supervision and guidance of a registered therapist (e.g., Physiotherapist or Occupational Therapist).
Deliver exercise programmes and functional activity practice in ward environments, adapting within agreed plans and escalating concerns appropriately.
Support patient flow by helping patients progress towards safe discharge (for example, practising mobility, transfers, stairs, and daily activities as appropriate to the pathway).
Accurately record interventions and patient progress in line with service standards and local documentation requirements.
Work closely with the wider multidisciplinary team, communicating clearly and promptly about patient progress, risks, and any changes in presentation.
Undertake administrative tasks to support the efficient running of the service (including routine admin, equipment/stock checks, and ordering within agreed processes).
