Full Job Description
As a Band 4 Senior Rehabilitation Assistant, you will work as part of a multidisciplinary team to support the delivery of rehabilitation programmes, helping individuals to achieve their goals and maintain independence. This includes working alongside therapists and other colleagues to provide coordinated, high-quality rehabilitation. You will also contribute to supporting discharge from hospital by delivering timely rehabilitation interventions in patients' own homes where appropriate.
You will support the management of a varied caseload under the direction of a qualified therapist, including patients with complex needs, and contribute to service development and quality improvement initiatives.

You will be supported by experienced community therapists and will follow guidance and, where appropriate, complete assessments of individuals in their own homes to maximise their functional outcomes.
You will receive regular one-to-ones, training, and the opportunity to attend peer support meetings with other Band 4 staff.

If you are ready for a new challenge and want to make a difference to people's lives, we would love to hear from you., To co-ordinate and conduct rehabilitation programmes (including 1:1 and group classes) for patients both independently and under the guidance of the Physiotherapists. This will initially be in care home settings and individuals own homes.To work closely with the physiotherapists and occupational therapists in the rehabilitation of patient with mixed physical diagnosis, recognising and immediately escalating any concerns around patient deterioration or lack of progression, referring back to physiotherapist or occupational therapist as appropriateResponsible for recognising signs of a potentially unwell patient while under their care either in class or 1:1 settingTo be responsible for the diary management of their classes/patientsTo be a resource for continued training to wider registrant and rehabilitation assistant workforce in the use of safe and effective exercise rehabilitation.
