Full Job Description

o Provide telephone-based and face to face support to users and carers who require interventions, often when they are in crisis and distressed.o Act as second person when completing a crisis assessment, with specific focus on social aspects of a patient's life.o Complete referrals and signposting to relevant agencies/services.o Deliver timely and specific mental health interventions as part of the 72 hour follow up off after a crisis assessment.o Use active listening and basic counselling skills.o Liaise with other colleagues in the team to discuss appropriate careo Adhere to all Trust policies with regard to Information Technology, confidentiality etc.o Assist in the design and implementation of service development projects within the service as required.o Attend meetings in which service developments are planned and discussed.o Participate in continually evaluating their own practice and the effectiveness of the service.o Participate in evaluating how users and carers perceive the
service they have received from the service.o Contribute to the development and maintenance of the highest standards, through active participation in both mandatory and supplementary training.o Maintain the highest standards of clinical record keeping and report writing in accordance with Trust policies and procedures and relevant professional codes of practice.
