Senior Reablement Assistant (Nights)

Want to make a genuine difference by helping people remain independent and avoid unnecessary hospital stays? The primary role is to support the delivery of a high-quality Reablement Service within the Intermediate Care Centre. This service aims to prevent unnecessary hospital and care home admissions while supporting early discharge from hospital.,

• You will work with individuals to help them regain confidence, independence, and daily living skills.
• You will support the delivery of a person-centred reablement service
• Assist individuals to regain or maintain independence in daily living activities
• Work collaboratively as part of a multidisciplinary team
• Monitor and support individuals with complex health needs
• Record and report baseline observations (e.g. vital signs)
• Assist with the supervised management of health conditions
• Contribute to safe discharge planning and ongoing community support
• Promote dignity, independence, and wellbeing at all times
  
  You will preferrably be Educated to Level 3 standard (or equivalent)
• Have previous experience in a care setting
• Have experience with administering medication
• Have ability to work as part of a team and independently
• Demonstrate good communication and interpersonal skills
• Require a compassionate, patient-centred approach
