Full Job Description

Are you an experienced Nurse or Matron who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 7 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing and leading efficient, integrated, urgent care to our patients with care and respect
Part of this role will include covering our EEAST Ambulance Stack and supporting patients who have called for an ambulance where appropriate.
These posts are for either our North (Bedford) locality or our Mid (Flitwick/Shefford/Biggleswade) locality. Preference will be asked at interview, You will be working as part of the Rapid Response team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage, prescribing (NMP qualification is essential for this role), and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.
Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions., You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community. You will deputies for the Service Manager as required to ensure Unplanned Care pathways are delivered to a high standard.

Integrated care is key in making the best use of resources and providing effective care.
