We are recruiting a Senior Physical Health Check Coordinator to join our Access Community Mental Health Team in Swindon to develop effective and trusting relationships with individuals who have severe mental health conditions such as bipolar, schizophrenia or psychosis, in order to support them to access SMI Physical Health Checks and support them to identify appropriate services and support to improve their physical health., We are committed to working with a recovery-based approach and supporting people to meet their individual needs.
