Full Job Description

This is a new role created to support the multidisciplinary team at Powys Living Well Service deliver a biopsychosocial approach to the management of long term conditions specifically: persistent pain, chronic fatigue/ME, long covid and weight management issues. People referred to the service will often be experiencing multimorbidity and have complex needs. There will be a focus on reducing unwarranted polypharmacy and ensuring that any medications taken are used in the most effective way., The Pharmacist role will be a key contributor to the Powys Living Well Service providing specialist expertise around medicines, building on the existing multidisciplinary team (MDT) members clinical expertise and supporting primary care colleagues and service users. Managing a clinical case load including agreed prescribing responsibilities.Providing expert advice on medicines and therapeutics, including specialist pharmaceutical advice and legislative guidance for the Living Well Service.Leading on policy and guideline development around medicines for the management of long-term conditions. Updating health care practitioners on changes in clinical practice; guidance and national policy around medicines within the therapeutic areas covered by the service.Supporting individuals who would benefit from clinical pharmacist input, including medication reviews, optimising medicines use and identifying opportunities for deprescribing and encouraging self-management.Contribute to teaching,
training and supervision of other staff within the service.Undertaking research, audit and service development will be an integral part of service delivery.
