Assess patients' requirements, in relation to their goals. Plan, manage and review functional and psychological restoration for individuals and groups. Review care plans at timely intervals and communicate effectively with the patient's GP, advising on ongoing and flare up management plans agreed with the patient. Provide safe, effective and dignified care according to best practice and national guidelines. The successful candidate will be required to work a hybrid working pattern facilitating face to face clinics in a number of sites in the Wolverhampton area, Pain Management Programmes both face to face and online and pain education. They will be joining a multidisciplinary team of physiotherapists, occupational therapists, nurses, psychologists and consultants who are committed and passionate about providing a biopsychosocial model of care to people with persistent pain.
