You'll work alongside people to overcome everyday challenges, build life skills and achieve goals that matter to them. What youll do Carry out person-centred assessments to understand how a customer's health, abilities and environment affect their daily life and independence Work with customers to set meaningful goals and create practical plans that support progress, confidence

and wellbeing Help customers develop skills, overcome barriers and take part in everyday activities at home and in their community Provide clear occupational therapy advice and guidance to colleagues, helping them deliver consistent and effective support Work closely with families, carers and partner professionals to make sure support is joined up and customer focused Manage a varied caseload, review progress and maintain clear records that support safe and effective practice Why join us This is more than a job. You have A degree in Occupational Therapy, HCPC registration and a strong understanding of occupational therapy and person-centred practice Significant experience of assessing people with physical health, mental health, cognitive, learning or communication needs
