Full Job Description
This is a unique and exciting opportunity to be part of an innovative new model of care (NMOC), and to use all of your clinical and interpersonal abilities, develop new skills, and work with men who need and deserve high quality care. Feltham B is now a Category C male adult prison and is currently aligning its services to the New Model of Care (NMOC). The NMOC is designed to promote patient safety and wellbeing from the point of reception into custody through to release, ensuring continuity of care, appropriate risk management, and seamless handover into the community or onward transfer to another establishment. The NMOC is structured around four core teams (pillars): 1. Early Days in Custody (EDIC) - focusing on immediate health, mental health, and substance misuse needs at reception and during the early period in custody. 2. Planned Care (PlC) - delivering ongoing, structured care for patients with identified health, mental health, and substance misuse needs. 3. Urgent and Emergency Care (UEC) - providing timely assessment and response to acute and urgent presentations. 4. Release and Transfer (RT) - ensuring safe, coordinated transition of care on release or transfer, with robust communication to community and receiving services. This model supports a whole-pathway, patient-centred approach, underpinned by multidisciplinary working, clear governance, and shared accountability.
