Full Job Description

We are seeking a motivated and forward-thinking Senior Occupational Therapist to join our dynamic Hospital at Home service. As a key member of our multidisciplinary team, you will play a vital role in delivering rapid, high-quality assessment and intervention for adults experiencing acute health needs, frailty, or exacerbations of long-term conditions.

Working autonomously, you will provide specialist occupational therapy assessment, care planning and treatment to support admission avoidance, promote independence, and improve quality of life for patients in their own homes and community settings.

In this role you will collaborate closely with Consultant Practitioners, ACPs, the AHP Clinical Lead, GPs, community teams, ambulance services and social care partners to deliver coordinated, person-centred care. You will contribute to complex case management, lead on therapy-driven interventions, and use your clinical expertise to guide the wider team.

You will also provide professional leadership within the team, supporting junior staff, students and unregistered colleagues through supervision, mentorship and role modelling of best practice. As part of our fast-paced service, you will work across single point of access and community caseloads, responding flexibly and compassionately to urgent referrals.
