Full Job Description
· To complete initial and ongoing patient assessments, support patients to identify SMART goals, utilise clinical outcome measures and develop, deliver and progress bespoke treatment plans.
· To work collaboratively within the team and with external providers to:
+
o Enable patients to work towards goals relevant to the in-patient community hospital setting
o Support the patient and family members/carers with regards to discharge planning
o Identify further rehabilitation needs to include onward referral to other services as appropriate
o Support in developing the service in line with national and local strategies
+ Demonstrate critical thinking in the clinical decision-making process. Evaluate the effectiveness of the treatment through autonomous and accountable decisions within OT practice supported by current research and in accordance with NHFT protocols and policies.
· Provide leadership, education and support to other team members through promotion of innovative strategies to manage change and improve service delivery
· HCPC registration and relevant degree/qualification in Occupational Therapy
· Good communication and organisational skills
· Manual handling experience
