Full Job Description
The Dementia Intensive Support Team (DIST) based in Bury St Edmunds, are excited to welcome applications for a Senior Mental Health Practitioner (Nursing) with experience and keen interest in working with people living with dementia or complex cognitive impairment. The DIST focus on independence and implement strategies which can avoid premature moves to care homes or hospital admission., Working in a busy and dynamic Community Team means every day will be different. You will be involved in (not an exhaustive list):
- Managing a complex caseload
- Undertaking triage duties and initial urgent/routine assessments on behalf of the service
- Attending multi-disciplinary team meetings with our Consultant Psychiatrist, Clinical Team Manager, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologist, and Clinical Associate Psychologist
- Undertaking face to face visits in the community setting including home visits and care home visits
- Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders including Integrated Neighbourhood Teams
- Involved in the clinical leadership decisions as part of the senior leadership team to make sure that KPIs are met and team goals are being worked towards
- Supervision and mentorship of junior staff members
