Full Job Description

The Band 7 lead Heart Failure Specialist Nurse is responsible for providing advanced clinical leadership and expert management for patients living with heart failure within the community setting. The post holder will lead and coordinate the delivery of a high-quality, evidence-based heart failure service, ensuring seamless care across primary, community, and secondary care interfaces. They will act as the clinical lead for the heart failure nursing team, supporting service development, clinical governance, and workforce supervision. The role involves independent patient assessment, titration of heart failure therapies (including non-medical prescribing), and patient education to promote self-management and prevent hospital admissions. The post holder will work closely with cardiology consultants, GPs, pharmacists, and the wider multidisciplinary team to optimise heart failure management, improve patient outcomes, and enhance quality of life. They will also take a key role in clinical audit, data reporting, and service improvement initiatives aligned with national heart failure standards and local commissioning priorities.,

  • Provide expert clinical assessment, diagnosis, and management for patients with heart failure in the community.
  • Lead and supervise the community heart failure nursing team, ensuring high clinical standards and effective caseload management.
  • Use non-medical prescribing to optimise evidence-based medication titration.
  • Act as the link between primary, community, and secondary care to ensure integrated, patient-centred care pathways.
  • Support education, training, and development of junior staff and students.
  • Lead quality improvement projects and contribute to service evaluation and redesign.
  • Participate in multidisciplinary meetings, audits, and data collection to monitor outcomes and performance against national guidelines.
  • Provide compassionate, holistic care, empowering patients and carers through education and support.
