Full Job Description

We are seeking a dynamic, highly motivated individual to work within the Enhanced Care Home Support Service, to support residents within a care home setting. You will be required to care for patients living within both nursing and residential homes, many of whom are frail, elderly with long term conditions and complex health needs. The role will involve patient triage and undertaking the clinical assessment and examination of patients with a view to supporting prompt diagnosis and treatment of care. The successful candidate will possess the clinical examination module or will be willing to undertake this module, and non-medical prescribing would be beneficial. The service works closely with the multidisciplinary team to deliver the Directive Enhanced Service in collaboration with the Primary Care Network. The post creates an ideal opportunity for an experienced nurse wishing to expand their skills and knowledge within this area of expertise.

RGN/RN Adult Diploma or Degree on the NMC Register Ability to work as a team player The post holder will be a named nurse for their identified care homes and will respond as appropriate and as defined by the priority/acuity of the caseload and liaise with all health care professionals and other agencies within the wider community to ensure effective service delivery. Knowledge of relevant NICE guidelines in relation to clinical practice The post holder will work under the direction of the clinical lead for the service and will be responsible for the assessment of patient need. The post holder will develop, advise, and evaluate programmes of care within the care homes. The post holder will support the care management of complex cases and further support leadership and development within the team.
