The Transfer of Care Hub (previously The Discharge Hub) in East Suffolk and North East Essex Foundation Trust has undergone a large amount of change in order to adapt to changing demands and improve the discharge process for our patients, and this transformation is continuing.   • To support and educate the ward staff and case managers to co-ordinate complex cases throughout the discharge planning process, in order to avoid unnecessary hospital stays and to ensure the safe and timely discharge from hospital over a seven day service.
  • To complete appropriate assessments ( TNA, MCA, CCG referrals ) in accordance to the DOH Framework for NHS Continuing Healthcare and DOH Hospital Discharge Policy and provide signatory for completion of Assessment of Nursing/Healthcare Needs.
  • To act as support for junior staff, and take leading role in ward staff and team's development regarding discharge process/pathways.
