Full Job Description

We are seeking to recruit a motivated, enthusiastic individual into this challenging but rewarding post.

The senior support worker will work closely with the community learning disability nurses supporting them with all aspects of care within the Service. With a particular focus on assisting with assessment , planning , implementing and evaluating care packages as designated by the multi disciplinary team

They will work closely with the community learning disability nurses to support the care pathway, with supervision and on-going support., You will be actively involved in the implementation of the agreed care / intervention programmes . working under the supervision of the community nurses you will support individuals in a variety of settings as well as within their own home .

The Doncaster Community Adult Learning Disability Team is a specialist Health team working to meet the specialist health care needs of adults with learning disabilities. The post will also involve working across the Doncaster Care Group to provide support to adults who have a diagnosis of learning disabilities.
