Full Job Description

+ Deliver high-quality, evidence-based nursing assessments, care planning and interventions within community settings. + Provide intensive, short-term support during periods of crisis to reduce risk and avoid inpatient admission. + Conduct comprehensive risk assessments and implement positive behaviour support and trauma-informed care approaches. + Act as a care coordinator and key professional within the MDT, liaising with families, carers, social care and partner agencies. + Lead on clinical decision-making, supervise support staff, and contribute to teaching and mentoring. + Monitor, review and evaluate care outcomes, ensuring care is responsive and recovery-focused. + Maintain accurate clinical records and ensure practice complies with RDaSH policies, safeguarding, Mental Capacity Act and professional standards. + Contribute to service development, audit and quality improvement initiatives., We welcome applications from individuals who may require sponsorship [if eligible] under the skilled worker or health & care visa route.
