Senior Community Nurse - Epping

The West Essex Specialist Dementia and Frailty Service (SDFS) is an assessment, diagnostic and support pathway for people with dementia of any age, as well as people over the age of 75 who have complex mental health needs and an associated age related frailty. We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach., To provide care co-ordination to a caseload of complex patients To complete safeguarding investigations To carry out care act assessments for patients under the age of 65 Will be part of the daily 'duty' rota supporting Urgent Community Response To work alongside the multi-disciplinary and multi agency team delivering high quality, personalised care.
