32 hours (4 Nights) working between 23:00 -07:30 over 7-day rota The Evening and Night community nursing team is a countywide team which is centrally based in Droitwich. This post is primarily based at Droitwich with the flexibility to work countywide. As part of the role, you will be required to be part of the triage team for some of your shifts, which involves taking calls from other health professionals and direct calls from patients and their relatives, overseeing the electronic referrals that come via EMIS and the west midlands Ambulance service system CAD. You will also have clinical responsibilities and visit patients in their own home supporting them in remaining at home and preventing admission to hospital and supporting early discharge. The post is all worked as unsocial hours hence picking up pay enhancements for all hours worked which makes this post more attractive. more money for less hours!! If you would like to discuss informally if this is the right role for you, get in touch with Recruiting Manager which can be found on the top right-hand side of the advert., You will be joining the Night element of one of our dynamic and supportive Neighbourhood Teams to provide a 24-hour admission avoidance service to the population of Worcestershire which encompasses both urban and rural areas. The successful candidate will deliver independently a high standard of nursing care for patients in their own homes. Ensuring that care provided maintains the individuals privacy and dignity, values diversity and respects cultural and individual rights and choices.

We are looking for candidates with good communication skills, a flexible approach and enthusiasm to work within the team. You should be a committed, highly motivated and forward-thinking individual who is a good team player. The successful candidate will also assist the team to develop and achieve team and organizational objectives. They will also participate in staff meetings and training & development activities as necessary to ensure their skills and knowledge are up to date. Due to the nature of the post the applicant must be able to travel independently. Whilst a base will be identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.
