Full Job Description

We are looking for an experienced and skilled practitioner to join our Home Treatment Team in Ivybridge. The team covers a large geographical area and is closely aligned with the West Devon Core Team, sharing the same management and clinical leadership This is an exciting time to join the Home Treatment Team as it undergoes a number of new changes to increase the remit of preventing admissions and facilitating early ward discharges,

  • Assessing referrals from primary care, psychiatric liaison, community mental health teams and the first response services
  • Working with patients and their families to prevent hospital admission and facilitate discharge
  • Leading the shift and planning work across a large geographical area
  • Referring onto other serivces as appropriate
  • Making safeguarding referrals as appropriate
