You will work within an integrated Children's Nursing team covering both CCN/HAH, the role as a community Children's Nurse is highly varied . We are one large team covering both borough's of Greenwich & Bexley. The caseload is varied with referrals of CYP who have complex medical needs, long term health conditions and short term health conditions requiring rapid service input to enable CYP to be discharged from hospital early.

An exciting opportunity has become available for a part time band 6 Senior Children's Nurse to work within the Bexley and Greenwich Community Children's Nursing Team and integrated Hospital at Home Team. The team provides a seven day service from 08.00 to 20:30, there is also 24 hour on call provision for children & young people who require end of life care at home. The CCN/HAH is part of an integrated network of services for children in Bexley and Greenwich providing highly skilled, nursing care to children and families with nursing needs in their own homes or community settings. To provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care. This is a post suitable for a Children's Community Nurse who should have experience of working within a Community Children's Nursing Team.
