Full Job Description

  • Conducting annual reviews and applying principles of positive risk taking, strengths-based approach and least restrictive practice to packages of care
  • Supporting the Multi-Disciplinary Team to screen and approve new funding applications
  • Participating in discharge planning meetings, Care Programme Approach and Care and Treatment Reviews
  • Providing proactive case management to individual complex and rehabilitation clients
  • Facilitating timely discharges from acute hospitals and placements where clients are no longer gaining benefit from that service

  • Be able to assimilate large amounts of information and have excellent verbal and written communication skills, with attention to detail
  • Ability to develop relationships and foster effective networks with key stakeholders, families and carers
  • Be able to assess and prescribe care dependent on the individuals holistic health and care needs
  • Have a strong value set around quality and patient/carer experience
  • Have excellent time management skills and thrive on working at pace
  • Have excellent knowledge of Microsoft office and have intermediate typing skills
  • Be local or be able to commit to regular onsite working and able to travel daily across the county and occasionally countrywide
