As a Children's Community Nurse (CCN), you will be crucial in delivering high-quality nursing care to children in their home environments. You will conduct assessments, develop care plans, implement nursing interventions, and ensure proper documentation and follow-up.

Main duties of the job
To work as an autonomous practitioner responsible for helping lead a team of Nurses to provide high-quality, specialist nursing care to children in their own homes.
To provide continuing responsibility for assessing, developing, implementing and evaluating nursing care for patients.
Work with other local health care professionals/providers to maintain high-quality care and promote family-centred practice.
To continue to develop and maintain close links between Community and Hospital based services.
To be responsible for supporting junior members and students by developing their skills and providing mentorship through their practice placements., 1. Improving representation at senior levels of staff with disabilities, from black, Asian, and ethnic minorities background, identify as LGBTQ+ and women, through improved recruitment and leadership development
2. Widening access (anchor institution) and employability
3. Improving the experience of staff with disability
4. Improving the EDI literacy and confidence of trust staff through training and development
5. Making equalities mainstream
