Full Job Description
As a Senior Case Worker, you will support adults with care and support needs to improve their health, wellbeing and independence through strengths-based reviews and personalised smoking cessation support. Working with people who may face barriers to accessing mainstream services, including those with disabilities, long-term conditions, mental health needs and complex social circumstances, you'll build trusted relationships and empower individuals to make positive, lasting changes that support a healthier future. Working within the Younger Adults Team, you'll collaborate with Social Workers, Public Health colleagues and partner organisations to embed smoking cessation into social care practice. Through compassionate, non-judgemental conversations, tailored interventions and effective partnership working, you'll help individuals understand the impact of smoking on their health and wellbeing, access stop smoking services and wider community support, and contribute to the ambitions of Smokefree Hampshire 2030.
- Strong communication skills, with the ability to engage people in sensitive, non-judgemental conversations about health and wellbeing that encourage positive behaviour change.
- Experience of working with adults with care and support needs, building positive relationships and using strengths-based approaches to motivate people to achieve their personal goals.
- Understanding of the impact of smoking on health, wellbeing, quality of life and wider associated risks.
- Knowledge of the Care Act 2014, Mental Capacity Act 2005 and how strengths-based practice can support positive outcomes and greater independence.
- Commitment to promoting positive health and wellbeing, championing smokefree lifestyles and empowering people to make informed decisions about their health.
Make a meaningful difference by helping people overcome barriers to quitting smoking and achieve lasting improvements in their health and wellbeing.
Support people through positive behaviour change, empowering them to make informed choices and take steps towards a healthier future. - Access to Health Assured's comprehensive Employee Assistance Programme to support your physical and mental wellbeing, including 24/7 telephone support, a suite of online resources, and legal and financial advice.
- Competitive benefits package, including generous annual leave entitlement, occupational sick pay, and access to the Local Government Pension Scheme.
