Senior Case and LDA Commissioning Manager

If you are enthusiastic and passionate about improving the lives of autistic people and people with a learning disability, then role as a Senior Case and LDA Commissioning Manager in our trust could be right for you. We have an opportunity for an innovative, skilled health professional or social worker to join our team who provide a commissioning service to adults and children with a learning disability and/or autism at a time when they need support from mental health services. This can be around preventing hospital admission and discharge planning, including commissioning community-based services to meet individual needs.,

• Provide case management support to autistic people and people with a learning disability who are at risk of hospital admission, or supporting discharge for these people who are currently receiving inpatient treatment., including identifying or commissioning services to meet individual need.
• Inputting into the referral and review processes for the Dynamic Support Register.
• Undertaking Care (Education) and Treatment Reviews

The service operates Monday to Friday, from 9am to 5pm. Although, weunderstand the importance of work/life balance and would welcome discussions around flexible working arrangements as required.
