Full Job Description
As a Secure Case Manager for the EoE Provider Collaborative, you will be working as part of the Quality Team to support the pathways for service users through secure hospital care.
You will be responsible for a caseload of adults who are inpatients in low, medium and high secure services and will lead on the quality oversight of a number of secure hospitals sites within the region., The case manager will have responsibility for oversight of the pathway of adults through low, medium and high secure services. Although the majority of placements are within the East of England, some service users are placed outside of the region in specialist services across the country and the case manager is expected to visit all individuals on an 8 weekly basis.
The post holder will also be responsible for leading on quality oversight of a number of secure hospitals sites within the region and will carry out regular visits as well as annual formal service reviews.
As a clinician within the TACT the post holder will work closely with the commissioning and transformation teams to ensure continuous improvement of the service.
