Full Job Description
- Work with the team to plan and deliver a commissioned healthy child programme 0-19.
- Identifies health needs by undertaking an assessment in partnership with clients, to deliver an individual programme of care, delegating support as appropriate.
- Keep accurate, legible, contemporaneous records and written reports as required
