Full Job Description

We work closely with our Primary Care Network, local providers, and wider gtd teams to deliver holistic care. We utilise an online triage system, with most patients initially assessed by telephone, alongside Enhanced Access Service to 8pm M/F to improve patient access, supporting flexible and effective workload management., Our patient population presents a wide range of clinical needs, offering a varied and rewarding workload. You will provide both routine and urgent care for acute and long-term conditions, working in line with local and national guidelines. Carrying out all relevant medical/clinical appointments including home visits and associated administrative duties within the practice such. The role will be at the forefront of delivering an innovative model of primary care service provision operating in partnership with other local gtd practices and the out of hours provider offering integrated primary care services. Provide medical coverage for the Enhanced Access Service, in agreement with medical team, which runs to 8pm You will embrace our values and ensure that they are instilled across all communications channels; we:
