Full Job Description

The Rough Sleeper Team provides dedicated support to individuals experiencing rough sleeping across Milton Keynes. We work directly with people who often face multiple and complex challenges, helping them move away from homelessness and towards safer, more stable lives. As a Rough Sleeper Navigator, you'll manage a caseload of adults with a wide range of support needs. You'll work closely with colleagues across the council and with partner agencies, building trusted relationships and coordinating practical, person-centred support. This is a rewarding frontline role where your work will make a real difference every day.   • Work closely with colleagues, partner agencies and other council teams to help reduce and prevent rough sleeping across Milton Keynes
  • Manage and prioritise a caseload of adults with complex needs, completing assessments and developing clear, strength-based support plans
  • Build positive, professional relationships with individuals experiencing rough sleeping, supporting them to engage with services and move forward
  • Work collaboratively with the Rough Sleeper Team and partner organisations to coordinate joined-up care and support
  • Maintain accurate and up-to-date electronic records, ensuring information is clear, timely and reliable
  • Take part in a rota to support outreach activity and provide a responsive approach during periods of increased risk or crisis

    Experience of working in health, social care or the voluntary sector, supporting adults with complex needs
  • An NVQ Level 3 in Health and Social Care is desirable, or equivalent experience gained in a similar role
  • Strong communication skills, with the ability to engage people using a strength-based and supportive approach
  • Experience of working alongside partner agencies in community settings
  • The ability to work well as part of a team, offering flexibility and support to colleagues when needed
  • Emotional resilience and a calm, professional approach when working in challenging situations
  • Full UK Driving Licence and access to a vehicle To be successful in this role, you'll need a full UK driving licence and access to a vehicle. If we offer you the role, we'll ask you to provide evidence of your driving licence and vehicle insurance, including cover for business use.
