We have agreatcommunity nursing opportunity at our Newhaven base in East Sussex. This postwould suitaRegisteredMental Health Nurse(but we will consider Learning Difficulties nurses who have sufficient experience of working with Dementia or Severe Mental illness.), The post-holder would join the existing Lewes intensive home support team who work to provide intensive home support for older adults in a state of acute mental ill health to prevent deterioration and need for hospital admission. This post will also help support the role out of the virtual ward into the Lewes neighbourhood mental health team, and is line with the Trust goal of preventing inpatient admissions for older adults wherever possible.

Please note this role is rota based and cover is required 8am - 7pm Monday - Friday and 9-5pm Saturday & Sunday

You would be a key part of our multi-disciplinary service for older adults with mental health problems, including Dementia. This is challenging work but we receive very positive feedback from our service users and carers and Sussex Partnership NHS Foundation Trust has recently achieved Outstanding for the care it provides.

Thispostwill involve working witholder People withcomplexmental health needs as well as physical health care needs. You will be expected to supportboththe clientandtheircarers.You will also need to be able to work cohesively with otherservices bothwithinourTrust, andwithotherpartneragencies.

You will need to becaring, compassionate and a good communicator,with a commitment toimproving the lives of our service usersthrough creative,evidence based practice. As a care co-ordinator, you will beresponsible forworking collaboratively with service users and their carerson your caseload to create care plans thatmeettheir needs and maximise their independence.

Youwill be based at Hill Rise,Newhaven but the ability to tavel is essential for this role.
