Full Job Description
This is a unique opportunity to play a pivotal role in supporting patients throughout their entire transplant journey - from assessment and preparation for transplantation, through to long-term post-transplant follow-up. Working across Norfolk and Waveney, including close collaboration with colleagues at the James Paget University Hospital and Addenbrooke's Transplant Centre, you will coordinate complex pathways, provide specialist advice and psychological support, and act as a key advocate for patients and their families. As a Renal Transplant Co-ordinator, you will be an autonomous practitioner and expert resource, developing strong relationships with patients, multidisciplinary teams and external partners. You will have the opportunity to further develop and expand our Living Donor Programme, helping to improve access to transplantation and supporting patients to achieve the best possible outcomes. Main duties of the job As post holder, your duties will include: You will manage a specialist caseload across the transplant pathway, coordinating assessments, investigations, follow-up care and education, while working closely with multidisciplinary teams, primary care and regional transplant centres. You will provide highly specialist clinical advice, lead nurse-led transplant and donor clinics, support service development, contribute to audit and research activity, and play a key role in maintaining high standards of patient-centred care. The post holder will act as an expert resource for patients and healthcare professionals, promoting evidence-based practice and improving patient outcomes. Examples of decisions made independently by the post holder include:
- Prioritising and managing complex transplant patient caseloads, identifying when urgent review, intervention or escalation is required.
- Planning and delivering nurse-led post-transplant follow-up care, including clinical assessment, patient education, referrals and ongoing monitoring to ensure safe and effective outcomes.
